Membership Forms
Membership Form - We now recommend you don't use this form but Register for Membership Online. This is an out-of-date membership form (Word doc) for you to fill in and email to us, print and send to us, or bring along to the club together with your subscription cheque. If you cannot open the Word document try this pdf Membership Form. Please refer to latest rates as those on the form are likely to be incorrect.

Juniors & Child Protection
- 2005 Junior Coaching Application Form - Download yours now.
- Parental Consent Form for Juniors - We now recommend you don't use this form as it is Now Online. To be printed and filled in by the parent/guardian for each child joining the Juniors coaching groups to inform the supervisors of all important information about you child.
- Junior Charter - A three-way charter between the Club, the coach and the parents outlining good practice for working with juniors.
- SCTC Child Protection Policy - Child Protection Policy adhered to by Sutton Churches Tennis Club in support of the LTA Child Protection Policy. Other sports clubs may wish to use this as template for their own policies.
- LTA Code of Conduct for all those working with Children in Tennis - Guidelines set out for the LTA. All our coaches, assistants and volunteers are familiar with these guidelines. Would also be useful for those working with children in other situations.
- SCTC Self-Declaration Form - All our coaches, assistants and volunteers working with children are required to complete one of these forms which is kept on record.
